If you are not able to complete these forms online, please arrive 15 minute prior to your appointment and we will be happy to assist you.

REMINDERS + NOTIFICATIONS

Appointment Details - Your appointment details will be confirmed via text, email and/or push notification through the vagaro app. If you did not receive this confirmation, please contact us to verify your appointment and contact information was entered properly.

Confirmation Request - 72 hours prior to your appointment you will receive a request to confirm that you plan to keep your appointment. Please confirm or contact us to reschedule at this point.

Appointment Reminder - 24 hours prior to your appointment you will receive text, email and/or push notifications reminding you of your appointment. At this point you will no longer be able to reschedule or cancel your appointments online as it is outside of our cancellation window.

WHERE TO FIND US

Conveniently located in the heart of Mandeville, just minutes from the causeway. You can find us in Magnolia Plaza, near Med Vet and Leonardo's.

Look for the Capital One ATM in our parking lot.

PARKING

Parking is FREE but can be limited between the hours of 11am-1pm Tuesday-Saturday due to Leonardo's lunch crowd. If possible, give yourself a few extra minutes to circle the lot during these times.
